Application process and initial order

Step 1

Review the Eligibility Criteria

Review the National EB Dressing Scheme eligibility criteria with your Approved Healthcare Professional to determine if you may be eligible. If you need a hard copy of the eligibility criteria, please contact Independence Australia.

Step 2

Obtain an Application Form from Independence Australia

Download an Application Form or contact Independence Australia to obtain a hard copy of the Application Form.

Step 3

Complete the Application Form and obtain all necessary documents

Step 3(a) Recipient or Authorised Representative completes Section 1 of the form. Step 3(b) Organise for your Approved Healthcare Professional to complete and sign Section 2 of the Application Form and obtain required supporting documentation. Step 3(c) Organise for your Treating Healthcare Professional to undertake an assessment and write a monthly dressing requirement on the Application Form selecting from the Approved List. This monthly dressing requirement will become your Standard Order.

Step 4

Send the completed Application Form and all necessary documents (including a copy of your Concession Card if appropriate) to Independence Australia
